To obtain a copy of your criminal history report in accordance with Alaska Statute 12.62, a social security number and either a state driver’s license or a Division of Motor Vehicle issued state identification number must be entered in order to verify your identity.
If these documents are not available, or you do not want to enter this information online, you will need to use the in person or the mail-in option to obtain a copy of your State of Alaska criminal history record. Additional information about these options or the background check request process in general, can be found online: https://dps.alaska.gov/Statewide/R-I/Background/Home.
To begin a name-based criminal history records request, please enter your current email address in the space provided and select submit. A secure link will be sent to the provided email address. Click on the link to be connected to the Online Criminal History Report Form.
